ACT! Training Interest Inquiry

Fill out the form below to indicate your interest in bringing ACT! Training to your team or club at no cost through the generosity of the Zac Plantz Foundation.

The U.S. Center for Mental Health & Sport will be in the Greater Chicago Area March 15 - 18, 2023.

Additional dates and locations across Illinois and Indiana can be arranged based on interest. 

The Zac Plantz Foundation will cover the costs of 6-8 mental health awareness training sessions. Get your club scheduled right away before the funds run out!

The training is intended for coaches, athletes (16 and older), parents, administrators, officials, and anyone else in your sports community.

The USC-MHS will be in contact with you shortly after you submit the training interest form to schedule the training.
